How to collect address for all employees based on pre-determined list?

    I am trying to get email addresses collected from our employee's.  I have a current list of employee names and want to put a survey out that would prompt the employee to select the name from the list and then would collect the name for that particular employee.

    - Is there a way to collect addresses for all of our employee's based on a pre-determined list of employee's?

    - Is there a way to reduce the list of employee names from the dropdown as an email address is entered?  This way the only names that remain on the list are the ones that need to be collected?

    - For a future survey that I am thinking of, would there be a way to have a user select their name from a drop-down, and then be required to enter a corresponding pin# to enable them to answer the next question?  The employee name and pin number would be contained in a spreadsheet or could be uploaded to the form, and then the idea would be that the user would select their name from the list (Spreadsheet Field #1), which would then prompt them for the corresponding pin number (Spreadsheet Field #2) and upon successful entry of the pin they would be given the ability to enter data in submission box (i.e. email address or another bit of information)

    I'm not sure I understood your question correctly. Do you want to add an address field in your form and collect the submissions?

    - Is there a way to reduce the list of employee names from the dropdown as an email address is entered?  This way the only names that remain on the list are the ones that need to be collected?

    You can use the appointment slot widget instead of a dropdown. By using this widget, you can limit how many times the email address has been selected.

    For your last question, maybe the spreadsheet to Form widget can help you.

    You just enter the key you have created and it will populate the form field.
