Form not capturing some fields

My form submissions are missing first and last name. I tested the form and attached is a screenshot showing the submitted form data. Next to "Ms." should read Johanna Wise. 

I cloned your form and tried to submit the form, the information did populate fine under Incomplete Payment information.

With your form, I don't see any submissions/incomplete payments today. However, the submission from 8/6 is displaying the name fine. Please clear the browser cache to see if it reoccurs.
