My form has been disabled on my website. When I tried to log in to my jot form account, I received a message that the account was suspended but I don't know why. Can you please tell me and reinstate?

Your account was suspended because of the Credit Card Authorization form you created. It is against our Terms of Use to collect credit card info using a plain text fields.

Phishing. You agree that JotForm, Inc. may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining, for example: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.

Card number, expiration date, security codes and card holder name are considered critical information and cannot be collected on plain text fields. The only services allowed to collect such data are those that are DSS/PCI certified.

I saw that you deleted that form, but the form is still located inside of the Trash folder in your account. I have Purged the form from the Trash folder and I activated your account.

If you want to have a payment field inside of your forms, you need to use one of JotForm payment tools.
