How can I get a complete PDF copy of a submitted form -- not JUST the answers, but everything that is on the original form?

As far as I understand, you need to include header and texts of your form on pdf report. If this is correct, please refer to the next guide: How-to-Show-Headers-and-Text-in-the-PDF
