how can I creat form

Hi,

At first, you should take a decision to use which form type. We have a two form type. One of that is classic form and other one is card form layout. In shortly, classic form is all questions in one page. On the other hand the Cardform layout is move on one by one. You can examine these types while creating form.

You can create using this steps: 

1-) Simply go to https://www.jotform.com/myforms
2-) You can see "Create Form" in the left top at page, click it.
3-) Select your form type.
4-) Then you should choose to new blank form or using template. 
5-) After that you will be created a form.
