Auto-suspended account

hi, i was using my account to create a form when suddenly my account kicked me out and when i tried logging in, it said "autosuspended". I would like someone to look into this for me please and have my account reactivated, thank you. I do not have a url of the page nor can i screenshot it since i was kicked out.

Your account is autosuspended because of the form Liability Waiver / Rental Agreement. I inspected that form but I did not find any fields that are violating our Terms of Use. 

It seems that our system suspended your account by mistake. I activated your account.
