Missing info when printed out

Some information previously on our forms are now missing for no reason. My boss has put them back. You can clearly see that they are on there, but when the client submits them and I print them out, that information is not there. Attached are the screen shots. Please help!!

May we know what information is missing in your email notification?

You can check your email notification email content and look for the fields that may be missing.

or you could delete the old email notification and add a new one. This will ensure you all fields will be in the new notification emails.

When I go to look at the form that a new client has entered, I do not open up the email. I go to Jot Forms, click on my form and then click on submission and print it out from there.

You could try showing headers and text in the submission settings.

I have checked your form and the Therapist network DT AT are question in a TEXT element.

Kindly follow the instructions above regarding "Show Headers and Texts"

and you should be able to view the TEXT element and print it.

Where do I go to find the "Show Headers and Texts"?

Go to the Submission panel.

Then in your submission panel, click the gear icon to open the submission settings and enable Show headers and texts.
