Why was my account auto suspended

I don't understand why my account was auto suspended I created a form that I cant access now...

Upon review of your account I found that it was automatically suspended by our Phishing Detector Tool (autosuspended), because of form: https://www.jotform.com/82256210962252 is requesting a password in plain text field.

Gathering this type of data in plain text fields will violate our Terms of Use, as this is only allowed through encryption along database administration; and our forms do not provide this type of method.

I have delete delictive field and reactivated your account.
