We have a form embedded in our site that seems to cause our customers problems with intermittent issues where the form is simply not submitted.

When discussing this problem with our customers, we can't seem to replicate the problem, however they assure me that they are completing all requisite fields.

I have cloned your form here and was able to complete it with no issues. Sometimes a form may fail to complete due to errors on the form. This can be fixed by scrolling up and filling in the fields correctly. The fields will be marked in red and show an error message.

Thanks for this. As I indicated, I haven't been able to reproduce the problem whenever we walk through the form completion. Our customers tell me that they don't get any message about incomplete forms, the form just simply goes blank.
I'll keep an eye on it and see what happens.

I also suggest you try re-embedding the form on your website using instead its iframe publish code.
Currently, the form was published on the website using its JS script code. It is possible that script conflict was happening between those scripts on your website and that of the form.
Using the iframe embed code will prevent the script conflict issues.
