How to create a form that you can draw on an image?

I need to create a form for nurses to use with patients.  One of the things I need to do is have a outline of a human body on one page and for the patients to circle the parts of their body that hurt.  They can then submit the picture to the server.  Is this possible with Jotform?  Thank you.

You can use the Draw on Image widget.

This widget adds an image of your choice to your form, and allows your form users to sketch on it - and the form is submitted with the image updated with what your users drew.
