name of a form.

Im publishing (as a direct link of your form).  Can I customize the 'tab' above the URL.  It currently displays 'Jotform', instead of the form name.

The form I am publishing is called .. 'Request an Appointment Form'.

That tab's names is Page Title and you can change there. Please follow this instructions.

1-) Click Settings tab in your form builder. 
2-) Select Form Settings tab in the Sidebar.
3-) Click show more then you can see Page Title section at the bottom of page.
