How to have certificate of completion after form submission

    Greetings...

I deliver online learning where the user will get a completion certificate template when the course is completed. the problem is, although the platform I use has this feature, I need a completion certificate that is numbered and signed by me. Do you have a solution whereas I can embed a link (to Jotform) at the end of the course where the user will complete a form and automatically after submission receive a signed certificate? If so, what is the cost for this?

    Thank you

    Dr. Gaines

You could create a form that can be submitted to you, for that you just have to create a Notifier: https://www.jotform.com/help/25-Setting-up-Email-Notifications

Then, when you receive such submission, you could approve it and sign and then submit it back to the user.

    For example:

    1) Add a signature field (or any other field that you need) to your form,

    2) Add Get form page URL widget:

    3) Create a condition that will reveal the signature (and other fields that you need):

Since with each submission, you receive an edit submission link, that you can use to edit that particular submission. When you click that link the above condition will be triggered and it will show the Signature field that you can fill and submit. You could create additional Autoresponder that will go to the submitter in that case. Let us know if that works for you or if you have any further questions.

    Thank you!

I would like a form that once the person completes the form and hits send button, the person either immediately receives the completion certificate or an email containing the certificate. The form would populate the certificate automatically and bear my signature.
    Example:
    [cid:6e4eb617-da7b-4c73-9bab-5c8e1766ef12]
The certificate has to be available within minutes of completing the form because the couple takes the form to get their marriage license. In other words, once set up, I do not want to be part of the process.
    Thank you for your help

You can add the Autoresponder Email on your form. The Autoresponder Email will be the confirmation email that the form respondent will immediately receive after submission.

User guide: Setting up an Autoresponder Email

Another feature of the Autoresponder email is that you can include file attachment in the email. So if you intend to send a copy of completion certificate document to the form respondent, you can do it using the Autoresponder email.

I hope this help. Let us know if you need further assistance.
