    Hi. I'm producing a gift certificate page/form for my web site.

    Can you suggest a template to use.

     

    #1 I need a recipient information form

    #2 purchasing/ puchaser form- which will connect into paypal

    and then a choice as to which person will then be sent the email, & gift certificate. which needs to numbered. There will need to be two return emails. One has to be a receipt info for purchaser, and the other has the gift certificate which may go either way.

    Ok, here we go.

    #1 so we have four specific gift certificates. i'm thinking to do four signup forms- for each. So on my web page I'll have four different BUTTONS for the client to access the form. How does that feel?

    If you mean four buttons and one form per button (not four), that would be fine. Would you like assistance with coding the buttons?

    Yes, but I dont know what that means and I have a couple of other questions first. for this form http://www.jotform.com/?formID=23426547901151, since I am going to have four specific buttons leading to four specific forms- can the Amount of Certificate just be a preset amount and how would i change that? Then as well the service title would be preset too. This form also needs to number each GC. I like the date selection at the bottom, but I guess do I need to put this into my Wix account to see how it functions?

    Okay, I need to clarify something first. Is it your intention to place four radio buttons on this form, each of which will connect to one (1) of four (4) other forms, one of which will be http://www.jotform.us/form/23426547901151? In other words, do you want to create a setup where if one of this form's radio buttons is selected and the submission button clicked, the user will be directed to http://www.jotform.us/form/23426547901151 but if a different radio button is clicked, the user will be directed to a different gift certificate form and so on? Or is it that this form is not meant to be part of the process at all and you were just testing something?
