Need help in creating an approval workflow form.

Please refer to the attached document

Cellphone number: I must add the current number as per my form - but the person can change it as he chooses all the other fields must work the same. Question: how do I add a field with the information beforehand and allow the person to change the details:

Email adress: This field is the same as above

Residents and non residents with fingerprint access under.....: i have to add the details of registered person already. they will not be allowed to remove the person, but there must be an option to choose if the person must be removed.

Daily Access: I must add the days as per my information, but the person can change the information in this field. How do I create such a field / spreadsheet?

The form must have the following capabilities besides the above

creating an if option : If the form is unchanged the person must accept the changes and submit

creating an if option: if there is changes the person chooses this option and will form will then be editable.

The person must then accept the changes: I can then check the database based on the populated field who filled in what and who submit no changes.

There must be an declaration of course

In phone field properties, look for the "Placeholder" to pre filled your phone number into the field.

Same goes with the phone number field, look for the placeholder option and input whatever email you want to be pre filled.

If you don't want them to edit the field, you can set a condition for that to enable or disable a field. As per the form I attached at the bottom there is a whole area with blocks with the persons name on the left and on the right there is blocks where to add the days, how do i accomplish this type of design of blocks?

For instance they made the changes on the form and email it, lets say for instance they want to go back and make more changes will the form be saved on the previous changes? And saves itHe will then see the current changes and amend it as he chooses, the form will show the last changes....I hope it makes sense

I still do not get around something.The user that fills in the form.submits itThe form must be saved somewhere for the user to go in and make the necessary changes again. In other words I add the form link to the webpage he goes in makes the changes and submits it. Inreceive the updated form. So if he goes back to that same link on the webpage (the one he completed before) Will this link show the updated information?Or will it show the same infomation, I want to enable the person to submit the changes on that link but can go back where the changes show as amended and update it again. We do this excercise every 4 monthsThey have to verify the info....

I have sent the link for the person to fill in the form, I've added the conditions how do I check wheter the form is now updated?Or where do I check where the updated form is now, with all the conditions added if the person decides to change his number for example where can he locate the updated form? The board wants the form on the website where they are informed to go and update changes or if they choose to at any timeThank you

This is the response from my boss
Under fingerprints you have those tick boxes which is already pre-populated as per what is on record that should not be able to be adjusted. (which at the moment you can tick)  If the person wants to make a change it should have another caption underneath to say say UPDATE or EDIT once that is clicked then you can untick or tick additional days , otherwise the no changes caption should be set on default (so you can just view) that way if someone does click on update you will be aware of the changes made. (to be fair the fingerprint is the most important as this is the one that we penalize people for by giving them a fine, if someone's cell number has changed etc etc it is important that you are notified but it does not result in a fine being imposed.

Unfortunately, I am not sure if I still can understand the task in full. But, there is an option to disable the fields (checkboxes) for the case when submission is edited.
