Forms are not displaying on wp website

Greetings, 

we have been using forms for a while, but now it is not working, maybe after the latest update?

I tried to use the latest iframe code an it is showing, yet signature gets cut off to the side

please advice how to adjust that or maybe there is better way to integrate form into wp now?

The signature field is out of the form borders on your standalone form as well. 

To resolve this issue, reduce the width of the signature filed from 640 to 600 inside of the field settings.
