POR QUE MINHA CONTA ESTÁ DESATIVADA?

    Senhores,

    Minha conta encontra-se desativada.

    Não considero que descumpri qualquer cláusula do termo de política do JotForm.

    Além disso, caso haja entendimento pelos senhores de descumprimento de cláusulas por alguns dos formulários de minha conta, solicito que informem qual; ademais peço que habilitem a conta para que eu possa realizar a alteração ou remoção do formulário.

    Upon review of your account I found that these forms: https://www.jotform.com/70602956637664 , https://www.jotform.com/72265176412656 and https://www.jotform.com/61875051039658 are requesting ID Numbers (CPF/RG), which is consider a phishing activity, without the mandatory widgets.

    I recommend to check our Terms of Use.  

    In regards to the collection of ID Numbers it is allowed (if needed) with the mandatory agreements and signature widgets:
