How do I fix this very weird text?

The Problem
Every-time a response comes in from my form into the Google Spreadsheet, it's shows up as this weird text. I know don't know what it means, it's like some coding I guess. I'm not to smart on that topic. I don't know what to do, and I'm trying to start up a same business of mine and I need this form. No other side does a good job, I don't want the form directly on my own site, and Google Forms isn't an option for us. So I honestly need some help with this.

Troubleshooting
I tried one more time to fill out the form and the same thing happen and it copied itself too. So I only filled out the form 2 times but I got 3 outputs. Weird. I haven't tried any other troubleshooting as I don't know what the hell this is and how to even start to fix it.
