Can I upload a current PDF template?

I'm looking to upload this exact PDF form /template. 

After this form gets filled out, can I send this form to a specified email address?  

Are you looking to send that PDF via email notification after submitting a jotform? If that is correct, you will find the option(option 3) to attach that file at the Advanced settings: https://www.jotform.com/help/26-Setting-up-an-Autoresponder-Email
