Upon review of your account I found that it was automatically suspened (autosuspended) by our Phishing Detector Tool due to the form: https://www.jotform.com/82274655116660 because of the word Password in Spanish in plain fields texts.

Since forms are created everyday for phishing activities such as: stealing credit card information, social security numbers, login credentials, child pornography, etc. Our Phishing Detector Tool automatically suspends every forms that mentions words related to those activities. 

Gathering this type of data in plain text fields will violate our: Terms of use, as this is only allowed through encryption along database administration; and our forms do not provide this type of method.

I have deleted the delictive fields from your account, I have reactivated your account.
