Having trouble with 'Submit' button

    Hi,

    I have no problem integrating the code into my website; however, I can't seem to figure out why the Submit button isn't showing.  All I see is the code itself, but the button isn't there.  Can you help?

    I checked your webpage and I can see that the Submit button is showing properly. I tested it with Google Chrome and Mozilla Firefox. Here's a screenshot:
