Why is my account disabled?

    I did check your account and found that it is currently suspended as your form "Debt Consolidation Loan Questionnaire" is suspected to be used for phishing activity. 

    I am escalating this thread to the concerned team. We will get back to you on this soon.

    Your account contains loans form. Loan forms are solely allowed for banking institutions or credit unions. To continue using our services you have to furnish proof of legitimate business. If you are a third party contractor, we will need you to furnish proof that you are in association with the banking institution.

    We re-activated your account. Make the appropriate changes to the form and continue enjoying JotForm. We will review your account again for compliance.
