    Hi.......my form is

    <script type="text/javascript" src="//form.jotform.com/jsform/3265805268"></script>

    and is at

    http://www.carrbridgegolf.com/tee_book.htm

An older version of the form had a text area field caled "Other requirements" (as you can see from the screenshot.jpg attached) but I've removed that and refreshed the code on the website.  The problem is that the field still appears on the website.  Can you help please?

    Regards
