Inconsistent behavior - רכב כיבוי - חילוץ

I am being told by users that they get different results in using this same form

sometimes they have the option to sign the form and other they don`t 

some say they where not able to take a photo

the email sent with the submission report comes in different form submission to submission

I know it does not sound like normal jotform behaivior.

I was wondering if you don`t mind running a test on this form to make sure there is no anomaly?

I have tested the form https://form.jotform.com/82252601753957 on 2 different devices and sent 4 test submissions. The behavior is consistent based on what options the users choose.

I did notice the signature field and the camera field are optional fields in the form. This means your form users can choose to fill them or not while filling the form.

Make these fields required fields to ensure consistency. This will mean all users have to take a photo and sign the form before it is submitted.

This option can be found in the question properties.
