Customizable QR code: how to generate code with several fields' values?

    Hi,

    I’m trying to setup a from system where participants can sign up and provide me contact and other information on on tablet, and on another screen I can see the results come in. I need to be able to tap/click on the new entry and have a QR generated with something like “Firstname_Lastname_uniqueID” then I scan that QR into our computer. 


    Right now now I only seem to be able to generate a QR code with just “Firstname Lastname” 


    Is there a way to do this within Jotform or using a 3rd party integration?

    Hello,

    We have a Unique ID widget to assign ids to form submissions, but this widget only creates a value upon submitting a form: How-to-Add-Custom-Unique-IDs-to-your-Form-Submissions

    As far as I understand, you want to generate qr code within your form. I can suggest to add a Random Value Generator to the form and to combine Name+Random Value via Form Calculation widget. This value could be used when generating QR code:
