    Can anyone please help with this? My account was auto suspended. This is urgent.

    We have unsuspended your account but we deleted the following fields in your DIRECT DEPOSIT AUTHORIZATION form. Unfortunately, we do not allow asking bank information on the form.

    Kindly refrain from asking this type of information to avoid this from happening again.
