I have this form that isn't showing all the fields I have indicated in the form. I need it to show everything, especially if two peopel sign up, I need both peoples info. Also, you can see that the field asks, "HOW DID YOU HEAR ABOUT US", and the form that we get back says, "T-Shirt Size"???? Can somebody please help out? Thanks.Page URL:

The Eupraxia Sign Up notification does not have all of the form fields included in the template message body. This is the reason why the sent notification message does not have all info similar to the forms submissions data.
You may need to delete this email notification, and just create/add a new one.
This will then include all of the form fields into the email.
