I am getting reports that when people fill out the form they have the following experience:

"There wasn’t an error. I completed the form, clicked the ‘submit’ button and page reloaded minus all form-fields but asking for a captcha code again.

I restarted the process, exactly the same thing happened but the second time I entered the extra captcha code. The results page that loaded displayed  a message stating that the application had been submitted but I know technology well hence the follow up."

    and

"Filled out the form, attached a PDF, entered the challenge-response word and hit submit.  After the upload was complete - the screen came back to what looked like another challenge-response screen - but no place to enter the challenge.

     "

Why is the submit form responding like this?  We are fearful that we are missing valuable submissions.

Have you made any recent changes to the form?

We made a number change along the way.  Recently the main change was an updated confirmation e-mail.

    My test worked fine.

I am wondering if this is due to some kind of replication problem after you had made a change.

Do you know if these reports of problems happened soon(ish) after you made one of these changes?

I am not sure if changing the form URL, (which we usually suggest for replication isssues) as the form should now be fully synched accross all JotForm servers.

Is it typical to ask for the capta text twice?

The problems happend days after the form changes were made.  I switched from teh javascript embedding to the iframe.  Do you think that could make a difference?  One person reported a javascript error recently.

I have seen the captcha being asked for twice, when the form id used on the form doesnt match whats on the server when its checked after the submit button is hit. It happens most often when embedding full source code, which isnt the case here, but could suggest some kind of synch issue after you had a made a change.

Yes, I was going to suggest changing to an iframe embed, it will definately help isolate any issue. Perhaps, there is some kind of javascript conflict with the form code and any javascript on your webpage which only becomes appararent on specific browsers. Changing to iframe embed will mean any potential conflicts like that are eliminated.

I have switched to iFrame and got another report of a user getting a blank screen on submission.  Any ideas on what could be happening?

So this is slightly differerent, he got a blank screen rather than being prompted to re-enter a captcha?

Do you know if he waited until the file he was uploading had finished?
