I dont know why my account is block it and now I can't see my forms.

Your account was autosuspended due to having a form "Login" that is suspected of used for phishing. Kindly note that Jotform doesn't support login forms (with username and password fields) since they can easily be used for phishing.
I have activated your account and now you can access it. Kindly edit this form or delete password field to avoid your account from being suspended again.
