Looks like my account is auto-suspended again.
I was trying to import a form from Survey Monkey. Not sure why the password field is being added as I am not using a password field.
Please unlock this and provide a link to instructions so I can continue to import surveys from Survey Monkey.

I think what happened is you might have had the Survey Monkey home page URL instead of a form URL when cloning. The only form in the home page is the log in form to it attempted to import a log in form instead of a different form. I removed the imported log in forms and activated your account. Your forms and account should be usable once again.
