cloning and how to get latest version on non hosted solution

    when i try and clone a form i get the message below, we have a premium account hosted on our own server any ideas?  Also how do I get the latest version of jotform so I can upgrade, we last upgraded about 12 months ago? do you have process documents for our tech guys to follow if there is an upgrade route.

    Error on query: INSERT INTO `forms` (`id`, `username`, `title`, `height`, `status`, `new`, `count`) VALUES(23442436184, 'corporatehealth', 'Clone of SSP Management Referral ', '4486', 'CLONING', 0, 0)
    Error returned:Duplicate entry 'corporatehealth-' for key 2
