Our website is www.vitaclaychef.com our back end is shopify.com
the contact form and warranty form do not communicate to Zendesk.
Our customer complain we did not reply. Please help to check.
Is this anything do to we did not pay our monthly ? FYI, we changed our credit card and did not see you charge us after March 2018.
Please advise at your earliest. Thank you.

Can you please share to us the URL link to your form?
I was not able to find an existing form on the website you mentioned. We need to know the form URL ID so we can check also.

Plus the warranty claim is not connected to zendsk.

Did you try to share any screenshot? Unfortunately we are unable to see the screenshot you have shared. I would suggest you to please upload the image again and we will take a look. I did check the webpage which you have shared and did not find any issue with that. The embedded form seems to work as expected and I was able to submit form successfully. Please check the screenshot below:
Do you mean to say that the data is not being sent to Zendesk? How are you sending the form data to Zendesk?
Please explain your issue a bit more in detail and we will try to help you.

Hi,
1. I am not sure why your website showed contact page. We used different computers and smart phones to check but the contact form did not show.Do you have any idea why ? Please help.
2. You sent the contact form, but our Zendesk did not receive it. In fact, the last contact form we received is 7/2/18.
3. Our website 's warranty form is still there, but our zendesk also has not received any since 7/23. I just tested it, and it still did not go through to zendesk.
Please help. Thank you.

I have checked your website link and I was able to view your form.
Can you try using other web browsers in viewing your website.
I have checked your mail logs and email notifications are sent.
Regarding Zendesk, you can integrate it with JotForm using a thirtd party integration Zapier:https://zapier.com/apps/jotform/integrations/zendesk/2656/create-zendesk-ticket-from-new-jotform-submission
