Form is not sending test email to recipient email address.

It seems you are clicking on the "TEST EMAIL" button of your email alert. Please note that test email button of email alert will send test email to account profile email address. If you want to send email to the recipient email address you have added in email alert, you will have to preview your form and click on the submit button of form to test your form.
