Is there a way to create a selection for N/A or doesn't apply for Scale ratings?

I'm trying to add a N/A for not applicable as a selection in the scale fields for ratings and surveys.  is there a way to create a selection for N/A or doesn't apply, etc... ????

You can rename the label to N/A for the lowest rating text in your Scale Rating Properties>options.
