Is there a way for me to have the selections populate into the Guideline/Process Question box in separate lines instead of one line?

That's basically how it is designed in the calculation output due to its main purpose where users used them in the query string for the population.

Unfortunately, there's no way to achieve that in the form builder. You can, however, achieve that when you manipulate the form source code of your form.
