Creating anonymous survey

    If you would like to create an anonymous survey, do not add sensitive fields (e.g. Full Name, Email, Address, Phone) to the form.

    The only identifiable information collected will be the IPs of the form respondents.

    You can assure this by setting up unique submissions, which you can restrict by IP address, or browsers cache, please check this guide: https://www.jotform.com/help/64-How-to-Set-Unique-Submissions-on-a-Form
