Cannot get form to submit

    when we try to submit a from we get an error msg:

    I was not able to replicate the issue you are having. I did a test submission in your webpage and it was successful. Here's a screen capture:

    Is this form embedded on a webpage? If yes, then please try to re-embed the form using the iFrame method. Here's a guide: https://www.jotform.com/help/148-Getting-the-Form-iFrame-Code.
