Adding Paypal payment to form

It is me again!!! Thank you for all your help so far. 

So I know I can connect my payment calculation widget to the payment field. And I also know how to set a minimum due amount. Can these two be combined? Example our insurance requires a minimum payment of $5 regardless of the actual amount due. However, it can also exceed the $5. So my question is there a way to set the calculation field to round up to $5 if less is actually calculated.

Try adding a new Form Calculation field in your form, which you can hide: https://www.jotform.com/help/434-How-to-Hide-Form-Fields 

Then create two conditions, one to input 5 if the result from your "Final Cost ($5 minimum required by Mutual of Omaha)" is less than 5:
