Account suspended

    I can't seem to login to my account (cosolidarityfund@gmail.com). What do I need to do to make it enabled again?

    I have checked your account and it seems you are suspended due to possible scam. The form that got your account suspended was Loan application form.

    We understand this might not be the case with your form,  yet we had to act according to the general rule to be in compliance with our Terms.

    To lift the suspension, you need to email proof of affiliation to support@jotform.com to a banking institution as brokers. Or do you have an email address with a valid domain/corporate email address?

    Hello!
    We are a grassroots investment club that have only just started. You can find us online at www.coloradocoops.info/colorado-solidarity-fund or on Facebook at www.facebook.com/cosolidarityfund.
    I used the form template that Jotform provided because I was trying to develop a way for co-ops to apply for a loan.
    Attached is a photo of our bank information. We are members of Public Service Credit Union.
