Submit button showing "please wait" and no response

after i submit, it showing "please wait" and no further action.

Unfortunately, I was not able to replicate the issue. However, I encountered a different problem. I opened your form using the direct link and when I clicked the Submit button, it is not submitting. It is not showing the "Please wait" message as well. 

Please try updating your Gift Registry widgets, here's how:

In the Form Builder, select the Gift Registry widget and then click the "Widget Settings" button. Inside the settings, scroll down and click the "Update Widget" button.

Is this form embedded on a webpage? If yes, then please try to re-embed it using the iFrame method. This will prevent script conflicts between the form and the webpage. Here's a guide: https://www.jotform.com/help/148-Getting-the-Form-iFrame-Code.

I have cleared your form cache as well so that your forms will be refreshed from our servers.
