When I view individual submissions, uploaded images appear on screen. However, when I use the Get PDF feature, the image is not included in the pdf. I would like to generate a pdf that is a "complete" record of my form. Thanks!
Can you try this?
Open the submissions for the form, and click on the grey cog on the right, select the 'Include headers & Texts' option.
I tried selecting the "Show Headers and Texts" option but that did not include the uploaded photo submission in the PDF. Any other ideas?
Also, is there a way to name the PDF, rather than the generic 14 digit number? For example, name it accoring to the name of the individual who submitted the form.
Unfortunately, these options are not currently available.
Images are not included in PDFs at the moment and the naming option is also not availlable.

I believe showing the uploaded image in PDF was always possible. You can go ahead and create a sample form and test it yourself. The submission PDF should display the uploaded images.
