I have checked the Payment Integration in your form and I could not see a Fee that was set up.
You can set up the 3% Card Processing Fee by following the instructions on the image below.

ARCC123Replied on August 26, 2018 at 5:43 AMIt was working yesterday. Please revert to an earlier version. I just
want some simple validation and this has turned into a total nightmare.
Best for me to revert back to the version before any validation on
earliest/latest time. There have been 100 revisions and 100 instructions
but none work correctly and it is becoming never ending.
Kindly revert back to the version of yesterday where the tax was added.
I can live without the earliest/latest time validation.

was added. I give up on my request to have the validation. Too
complicated and none of the versions worked.
Sorry I can not follow your instructions and don't want to do any more
changes or have any more instructions.
I just want it working as it was previously with 3% credit card fee added.

If your form worked properly on August 16 or 24, you may revert to one of these versions.
Please let us know how it goes.

ARCC123Replied on August 26, 2018 at 9:43 AMI have reverted back to Aug 24 and Aug 16 but it does not add the 3%
credit card fee. There is something wrong with the revert function.
Kindly address this issue as it is in an endless loop. Please take the
necessary action so the 3% credit card fee is added.

The 'Credit card fee' appears to be already added.
