The Jotform's Google Maps functionality is no longer working.
Through my investigation of the issue, I discovered that, in June 2016, Google changed its terms of use policy requiring an API key in order for maps to show.
It appears they have given a two-year grace period before cutting it off. “Beginning on June 11, 2018, you’ll need to enable billing with a credit card and have a valid API key for all projects.” Google Maps Platform - Guide for Existing Users states (https://cloud.google.com/maps-platform/user-guide/)
I've included a screenshot of a submission with the mapping error.
