Autoresponder set to send email to patient completing the form

    Form hhttps://form.jotform.com/mhvance/lrmc-studentpatient-registration i have set the autoresponder to send an email to the email the patient's email address, I am not sure of my error. Any information with this issue will greatly appriciated.

    I can see that you have setup an autoresponder correctly, so it should be sent to the email address that is entered in the "Parent/Guardian's Email Address" email field.

    Is it not working? If it isn't, can we try doing a real submission in your form in order to test it?

    Thanks for your reply, the designated email was sent to the appropriate email address after a submission was done.
