PDF file does not load properly

I need this to work as a PDF file on iPads for our home visitors. They will not have access to wifi or internet and will need to use the form numerous times during the day and save the different info from each visit as seperate files to be emailed when they do have access to wifi or internet.

I cloned your form and I was able to convert it into a fillable PDF. Please try downloading it in the Form Builder, here's how:

In the Form Builder, click "Publish" in the top menu. Go to "PDF" section and then click the "Download" button.
