I'd like to know if I can customize one of my forms (82256657484165).
Currently it has three options for choosing reason for appointments.
New, return and procedure.
Is it possible to modify the form so that NEW and RETURN selections only go to one email recipient while the option of the PROCEDURE selection goes to a secondary email recipient without having to create 2 separate forms?

Yes, it is possible by using a conditional email sending, please check this related guide: https://www.jotform.com/help/167-How-to-Send-Email-Based-on-User-s-Answer
So you can specify which selection will trigger which email.
