Custom URL change problem

    Hi,

    I need help changing my service request form back to a custom link.

    Instead of https://form.jotform.com/52380261952151

    I want it to be https://form.jotform.com/servicerequest

    Before this happened, I looked at the details for hide a page in "conditions". I didn't change anything so may be a coincidence.

    When hitting edit where the link is shown, the option to apply shows and won't let me edit.

    You can set custom URLs in a predefined structure. It is like:

    https://form.jotform.com/UserName/FormName

    So your custom URL can be:

    https://form.jotform.com/Pawsibilities/servicerequest
