Can this be used in a Physical Therapy office to allow people to fill out forms on line or from a computer or IPad?

Yes, you can fill out the forms on the computer, tablets or smartphones. You just need to open the form using an internet browser. Here's a guide on how to get the direct link: Where-to-Find-My-Form-URL.
If you are planning to use it offline, then you can create a fillable PDF form and download it to your local device as a PDF file. You can open it using a PDF reader. This means that you can complete the form offline since it is a PDF file.
However, a network connection is still needed when submitting the form. Please refer to these guides:
