Can this be used in a Physical Therapy office to allow people to fill out forms on line or from a computer or IPad?

Yes, you can fill out the forms on the computer, tablets or smartphones. You just need to open the form using an internet browser. Here's a guide on how to get the direct link: Where-to-Find-My-Form-URL.

If you are planning to use it offline, then you can create a fillable PDF form and download it to your local device as a PDF file. You can open it using a PDF reader. This means that you can complete the form offline since it is a PDF file.

However, a network connection is still needed when submitting the form. Please refer to these guides:

Please take note that we are HIPAA compliant. For more details please check this link: https://www.jotform.com/hipaa/.

    Hope that helps. Thank you.

There are many medical offices that use JotForm.  We offer HIPAA compliant forms for that type of usage:
