I've checked the SPAM filter and junk mail with no luck. When this online form is filled out, the submission appears in the submissions list on the jotform site, but no email is delivered. I can send a test email successfully from the jotform site and those emails (from firstname.lastname@example.org) are delivered OK.
The form requires the user to enter an email address, and the emails have always "appeared" to be from the user's email address. I have created test form submissions and used a real email address of a user who completed the form on 12-5-2012. That submission on 12-5 was emailed successfully, but my test submissions using that email address are not delivered (and don't show up in SPAM or junk filters).

It seems that you are using a Postini mail service for your domain. I think that you need to whitelist our addresses via Postini Approve Senders feature.
Please check the following thread:
This will be fine for your current setup with email@example.com sender and custom Reply-to Email.
However, if you would like to use custom sender, you will need to whitelist our IPs instead.
