We have a problem with the form representation. Some of our client have this issue and can't order. I send you a screenshot that you can see what we mean with "the fields are shifted"
We think it's since we have changed the form from "script" to "Iframe" because of the required fields, but we are not sure.
Could you please help us?

Your form looks great on Chrome, FF and IE. Can you try clearing your IE browser cache and if this helps, please advice your users to do the same.
Also try to start IE with addon disabled if the above suggestion don't work and see if that makes difference.
Have you tried viewing the page to the other browsers?
