We've been using JotForm for over a year on our "contact us" page to submit emails from clients but have not received any submissions since September of 2012.
I personally did not build the form ... it was done by a tech from Network Solutions so I don't know what account with you it would be under.
So I can't check to see where the submissions are supposed to go. Plus I'm not familiar with JotForm.Page URL:

Sorry for the inconvenience that may have caused. As I've check your form notification recipient email address, I found out that this email address "info@sidstevens*******.com" is in our bouncelist and I will remove it for you.
Reason(s) : 5.3.0 - Other mail system problem 554-'5.7.1 The message from (<email@example.com>) with the =subject of (New submission: Click to edit this text...) matches a profile t=he Internet community may consider spam. Please revise your message before =resending.'
In addition, the said form was created under guest account and associated to this email address "info@sidstevens********.com". If this email address is yours, I would suggest you to register to Free account and used the same email address so that all your forms and datas would automatically transferred into your free account.
